The Hutt Valley District Health Board seeks a qualified andregistered social work professional for our medical ward.
You will be a key member of the multi-disciplinary team, providingcomprehensive psychosocial and risk assessments, and ensuring safe andsupported patient discharges.
This is an exciting, fast paced, skilled socialwork role. You will be working withpatients and their family / whanau who are experiencing varied and often complexemotional, social and physical health related needs.
The role requirescreative, innovative thinking and problem solving.
This position is part time and consists of 0.5 FTE (20 hours) perweek across Monday to Friday. You will workwith the other social workers on the medical ward to triage needs, allocatework and provide cross cover as required.

The Social Work Service is a supportive team, committed toprofessional development and continuous learning and clinical improvements.
Asa social worker within the service you will also provide cover to other partsof the hospital when required.
Moū About you
The preferred applicant will have previous health experience, willbe able to work comfortably within multiple systemic layers for the benefit ofour patients, and have experience supporting patients and families with complexdecision making and discharge planning.
You must have excellent communication and interpersonal skills.You must have excellent time-management and triaging skills, be self-motivatedand work well under pressure.
You must be a team player and be skilled indeveloping relationships with a variety of hospital based health professionals,in addition to community based services and primary health care providers.
