    You will work with our team based at Richmond HealthHub and have close working relationships with Nelson Hospital, other healthagencies, community services and Iwi / Health providers.

    We will value yourcommitment to working within a restorative model and your experience of workingwith older people with a health related disability or those under the age of 65affected by complex health conditions.

    This is a permanent 1.0 FTE position working 80 hoursa fortnight.

    You will be a motivated andenthusiastic registered health professional with a full drivers licence, goodcomputer skills and experience working with people with health relateddisabilities.

    Our team works with people to identify their goals and assist themtowards safe, independent living so a willingness to work within a partnershipmodel is essential.
