About the opportunity
Providing care plans and pathways to independence is at the heart of what we do. In this role, you will be working with older people and those under 65 years with long-term chronic health issues throughout the community.
Our Clinical Needs Assessors aim to help restore clients’ independence in the home setting based on the Restorative model of care.
Our Clinical Needs Assessors complete a range of electronic assessments with clients and work alongside family / whanau to ensure the best outcomes for clients.
About you
You will be a registered health professional, with a current annual practicing certificate (APC). Our Clinical Needs Assessors need to have strong organisation and time management skills.
In order to be successful in this role, you will also need to demonstrate the following :
Previous use of interRAI is preferred, but not essential as training can be provided.
