About therole

This 0.9 nursing role sits within Te HerengaTangata, our Community Older Persons, Rehabilitation and Allied Health team.

It’s a great opportunity for someone interested, not only in developing nursingpractice in the area of gerontology, but who is also keen to innovate and whowill work in an inter-professional way where this will better meet the needs ofour people in the community.

We are looking for someone who can offer specialistnursing advice, care and expertise using a Comprehensive Geriatric Assessmentmodel and who will also use rehabilitation / patient centred principles inintervention planning.

The person in this role is a key member of the team, workingclosely with a range of professions, particularly Social Workers andGeriatricians and linking with the Nurse Practitioner to continue developmentof nursing within the service.

This CNS will also support the excellentclinical work being done within our Early Supported Discharge team.

Our community services are evolving and thisposition is also likely to evolve in the future, including with considerationbeing given to prescribing as the service may require this.

You will find professional support from the NursePractitioner and ADON Community and Primary Care, with day to day support fromyour interdisciplinary team, and Team Leader, together with ready access to theCommunity Health Nursing team also on level 3 of the Community Health Building.

Skills andexperience

  • Expertlevel on the Professional Development Recognition Programme (PDRP) or workingtowards this.
  • A minimumof three years’ full-time equivalent practice in the area they intend toprescribe, with at least one year of the total practice in New Zealand or asimilar healthcare context.
  • Experienceof working in a Community setting and current driver’s licence.
  • Experienceof working in an Interdisciplinary team
  • Advanced clinicalassessment and knowledge of : pathophysiology, pharmacology, assessment anddiagnostic reasoning in relation to the clinical management of, andprescribing for, patients with long-term and common conditions in New Zealand,within area of specialty.
