Disability Support Link Older Persons and Rehabilitation

Fixed term until February 2021 80 hoursper fortnight

  • DisabilitySupport Link is the Needs Assessment and Service Coordination agency forWaikato District Health Board. We are comprised of two teams;
  • the Healthof Older People team and the Disability Support Services team. We workclosely with other older person’s services both in the hospital and the community.

    Weare looking for a registered health professional to work in our Health of OlderPeople team. This will be a varied and exciting role that will workalongside clients, families, local communities and inpatient settings so thatthe process of a needs assessment and service coordination reflects bothcommunity strengths and client needs.

    Thesuccessful applicant will need to be prepared to work flexibly, learning andworking in the both inpatient and community roles.

    Weare seeking a Registered Health Professional, who holds a current New Zealandpracticing certificate, and has :

  • proven assessment skills including comprehensivegeriatric assessment, excellent written and oral communication skills
  • an enjoyment for working in partnership withpeople of all ages and their families to develop an individualised supportpackage
  • an ability to work with a multidisciplinary teamand across all sectors
  • previous experience working in the disability orgerontology sector
  • a current full New Zealand drivers licence
  • Priorexperience working with interRAI is an advantage but not essential as trainingwould be provided. Please to view the position description.
