Amber Care Facilitator (Full time) Palliative Care Service, CM Health-CMH20191469 Full Time permanent Working Monday - Friday

The standard of care provided to patients at the end of life is a reflection of the overall quality of health services, delivered to often complex patients with a range of needs.

These individuals are frequent presenters to hospital especially in the last year of life and it is important that all services provide a high standard of care at this critical time.

The overall objectives are to promote excellence for patients with uncertain outcomes and ensure shared decision-making in order to optimise quality of life.

The role of the Facilitator Amber and End of Life Care is to work clinically with the multi-disciplinary teams to support serious illness conversation, advocate for staff and patients and to develop the provision of education workshops and the use of resources associated with End of Life Care.

Are you a health practitioner with a passion for improving care for patients who are heading towards the end of their lives?

We need a dynamic individual with outstanding communication and training skills to work with our ward teams to help them understand and embed practices for identifying and managing patients with uncertain recovery, and those at the end of life.

We are looking for someone who can really get alongside the multi-disciplinary teams to support our goal of an improved end of life care experience for patients and their families / whaanau.

If you have an ability to persuade, encourage, educate and motivate and are not afraid of having difficult conversations along the way, you could be the person we’re looking for.

You will be supported by a group of committed cross-organisational staff and will enjoy all that Counties Manukau Health has to offer.

Essential :

  • Tertiary health qualification (medicine, nursing, allied health)
  • Experienced educator / trainer
  • Passion for improving end of life care
  • Some flexibility to work after hours occasionally if required
