Aroha Pūmau (with compassion); Mauā kia Tinā (better, best, brilliant) and Rāranga Tahi (connected).He angitūtanga : The opportunityThe purpose of the role is to co-ordinate care of a defined caseload of Māori and other patients with diabetes and supportUsing Māori models of care based on manaakitanga, whānaugatanga and arohaadvanced nursing specialist diabetes knowledge and expertise to provide and support needs across the community inpatient continuumTo work with the diabetes nursing team to support their care of Māori patients so they experience culturally safe pathway of careFacilitate and coordinate with primary care, NGOs and iwi so that Māori people receive the best care possibleNgā He korero mōu : What we are looking forExperience in : Managing patients with diabetes and complex health needs (preferred)Strong skills and experience in the use of Te Reo Māori me Ōna Tikanga in health settings.
Proven experience in working with tangata mauiui and their whānau and has the ability to understand complex whānau dynamics.
