We have a vacancy for a motivated andenthusiastic Registered Social Worker to join our inpatient multi-disciplinaryteam.

You will be working with anexperienced team who strive to provide high quality care and support to tangatawhaiora and family / whānau.

Youwill be expected to undertake comprehensive assessments and to contribute tothe interdisciplinary formulation of treatment goals and individualised plansin partnership with tangata whaiora and whānau.

Working alongside the clinical coordinator,nursing, allied and medical colleagues you will have a key role in supportingthe client pathway and discharge.

An ability to work as part of team isessential for this role. Experience of working alongside people with moderateto severe mental health or addiction issues is essential also.

Knowledge orexperience of working with older adults is desirable but equally this rolewould provide you with an opportunity to enhance or develop specialistknowledge and assessment skills.

About the Service :

The Mental Health Service Older Persons (MHSOP) ispredominantly a community based service with a 16 bed specialist inpatient unitlocated on the Kenepuru campus.

Ourservice sits under the Mental Health, Addictions and Intellectual Disabilities Directorate and works closely with other local and regional services within the3DHB area.

The purpose of the service is to provide comprehensiveassessment, treatment and ongoing case management, working in partnership withclients, whanau, formal and informal care givers and community agencies.

MHSOP provides assessment, treatment and rehabilitationservices for individuals presenting with :

  • Recent onset functional illness over the age of 65.
  • Dementia with significant behavioural andpsychological symptoms (BPSD)
  • Duties :

  • Provideeffective, quality social work services to MHSOP clients
  • Actively participate in the multidisciplinary team
  • Providecomprehensive assessments and plan treatment collaboratively with the client and their Whanau
  • Beactively involved in the treatment and recovery of clients.
  • Rolemodel and provide sound clinical and cultural considerations in themultidisciplinary team process.
  • Assistthe client and their family and whanau to achieve optimal health,well-being and safety through provision of appropriate assessments, treatments, rehabilitation and education.
  • Workclosely with community agencies to achieve the outcomes agreedby the multidisciplinary team process.
  • Skills & experience

  • NZ registered Social Worker with a current annualpracticing certificate
  • Minimum of 2-5 years clinical experience in an applicablerole
  • Self-motivated in developing clinical and professionalpractice
  • Active involvement in the professional development pathway
  • Commitment and understanding of the Treaty of Waitangi anda willingness to workpositively in improving health outcomes for Maori.
  • Current full NZ driver’s licence (required for roles basedin the community or where the role may be required to work across multiplesites).
  • A high standard of written and spoken English.
