The Specialist Diabetes Service is a multidisciplinary team that provides direct care for people with diabetes who have complex needs that cannot be met in general practice.

Specialists will often have a role in coordinating population health strategies, facilitating education and training, leading and supporting new patient technologies, supporting primary and community care diabetes teams, research and surveillance of complications.

This is your opportunity to join the dedicated teamat the Outpatient Diabetes Specialist Service as a Registered Nurse specialising in thediabetes management of the adolescent population.

The position is a 1.0 FTEfixed term position for a period of up to 12 months. You will be part of teamenvironment focused on empowering patients to manage their diabetes.

We arelooking for a passionate and empathetic team member who is :

  • A Registered Nurse with recent experience in themanagement of Diabetes, with an interest in developing your nursing practice toimprove health outcomes and the management of Diabetes within the adolescentcommunity.
  • Registered with the Nursing Council of New Zealandwith a current APC and mental health scope of practice.
  • Ability to work collaboratively in a multidisciplinary team.
  • Excellent interpersonal skills, ability to build effective relationships with a strong patient focus, passion for building knowledge and empowerment for self-management
  • Comfortable with technology and its use to support self-management and improve health outcomes.
  • A postgraduate health assessment qualification is desirable.

    The Diabetes Adolescent Registered Nurse will :

  • Function in a collaborative model with thehealthcare team to provide diabetes assessments, recommended treatments,patient and staff education, co-ordinated care and referrals to other serviceas indicated
  • Apply nursing knowledge, clinical inquiry skillsand critical thinking when determining care of patients with diabetes andassociated complexities
  • Contribute to processes, pathways, policies andprocedures to ensure safe, quality care to people with diabetes in theoutpatient setting.
  • Provide comprehensive care to patients usingtechnologies to manage their diabetes and optimise their health outcomes.
  • Your support and empathy accompanied with your excellent interpersonalskills and ability to build effective relationships with a strong patient focuswill see you succeed in this exciting opportunity.
