Exciting opportunity for an experienced Social Worker. This is a niche fixed term role for 12 months supporting people and whanau living with the effects of rheumatic fever.

As an ideal candidate, you will be a registered Social Worker with clinical experience working in multi-disciplinary teams in the provision of holistic, family centred and community based approaches to care and chronic condition management.

You will be providing care to adult patients with Acute Rheumatic Fever (ARF) / Rheumatic Heart Disease (RHD)and assist with solutions to their life complexities in a whanau and patient centred approach.

Adult Community Services includes Allied Health, Community Rehabilitation, Intermediate Care, Mobility Solutions, Specialist Nursing, District Nursing and Needs Assessment and Service Coordination.

There are approximately 230 clinical staff in community services that are focused on providing quality health outcomes.

  • The Social Worker will work closely with other directorates, services and agencies who provide nursing, social and allied health support to patients and families living with ARF / RHD including Noho Ahuru Healthy Homes, Whanau Ora, Starship Community, District Nursing, Cardiology Services etc
  • The role will work directly with a Clinical Care Co-ordinator in a culturally safe manner to assist with navigation of patients to holistic care and best outcomes assisting with removing barriers to healthcare and chronic condition management
  • There is a requirement to work with the Senior Leadership Team in reporting activities, outputs and outcomes from the service to ensure agreed key performance indicators of the program are met.
  • This is a full time, fixed term position for 12 months.
