Te Tumu Waiora - te reo for to head towards wellness’ - is a new model of primary mental health and addictions care and support which aims to provide all New Zealanders experiencing mental distress or addictions challenges with access to convenient, high quality, integrated and person-centred care and support.
The model is being rolled out nationally over the next four years.
The model puts mental health and wellbeing at the heart of general practice with focused roles, Health Improvement Practitioners Health Coaches and Support Workers, working as part of the general practice team.
These roles aim to connect directly with patients to assist them to make any necessary changes to improve their health and wellbeing and work towards an achievable healthcare plan.
This full time Support Worker role working Monday to Friday 8.30am- 5.00pm is based in the North Canterbury community working closely with the Health Improvement Practitioners, Health Coaches, and General Practise (GP) teams to ensure people can access the full range of help that they need.
The base for this role may change within Canterbury.
We are seeking an experienced Support Worker to provide a responsive and flexible service, working autonomously and as part of the Pathways Te Tumu Waiora team, and has good local networks across cultural and other community services or can build these quickly.

You will be reporting to the Team Coach and joining our Te Tumu Wai Ora access and choice primary wellbeing services provided in GP practices, working with Support Workers and Health coaches who all have the common purpose to support people experiencing mental illness.
We are interested to hear from people who have a caring nature and work in a recovery focused way, you will be able to provide medication support for our people when needed and have impeccable attention to detail.
Ideally, you’ll have some knowledge around community supports and how to navigate and access external services for the people we support.
