Broad Function
The partnerships & communication assistant director will be responsible for developing and implementing Woodlands Health’s partnership strategy in order to establish a care network with community, social and long-term care partners.
He / she will also be required to plan and execute communication initiatives in support of Woodlands Health population health efforts.
Key Responsibilities & Duties
Specific
Review partner provider landscape and asset mapping in Woodlands Health’s catchment region, focusing on community, social and long-term care services
Form a deep understanding of the community provider ecosystem
Develop and implement partner development strategy and framework to engage and enable the partner providers for population health programmes
Establish and grow partnership network with regular engagement platforms to engage the ground, enable clinical / operational collaboration, build a community of practice and to provide a feedback loop for the various implementation
Develop and implement framework for partner management, including the development of partnership agreements
Develop and maintain a community partner database as part of knowledge management
Develop and implement toolkit to systematise partnership development and to enable objective and consistent assessment of capacity / capability.
Lead planning and implementation of partner engagement events and activities
Collaborate closely with internal and external stakeholders in the planning and implementation of marketing and communication initiatives related to population health
Collaborate closely with internal and external stakeholders in the development of content for the various marketing and communication material using different mediums and channels
Work closely with Programme and Operational Leads to optimise results from partnerships with community providers
General
Support the Executive Director and Director of the Regional Health Office in overseeing the operations of the Regional Health Office
Participate in or support organisation / department projects as assigned
Any other duty assigned by supervisor
Job Requirements
University degree in any discipline from a recognised institution
At least 8 years of working experience with partnerships and / or communications-related experience
Healthcare experience will be an advantage
A strong team-player who is self-motivated and able to work independently in a fast-paced environment
Excellent interpersonal and communications skills
Resourcefulness in problem solving and resilient in overcoming challenges
Strong organisational and event management skills, with a keen eye for details
