    This role is working for Capital & Coast District Health Board - based in Wellington and working with people and their families in the Community for the ORA Wellington Community Team.

    This team works with people aged 16 and above with acquired neurological, long term, or age-related conditions including stroke, dementia, frailty and post trauma conditions

    About the role :

    This opportunity is for a Liaison Nurse to join our community service in a fixed term role until June 2019, 32 hours per week, days and hours will be negotiated with the successful applicant.

    You will work within an interdisciplinary team that works collaboratively and takes a patient centred and goal focused approach.

    As the Liaison Nurse in the team, you will provide comprehensive nursing assessments and intervention with patients mainly in their own homes.

    You will liaise closely with individuals and families, team colleagues, the inpatient service, primary care and other community services as well as the Geriatricians who work alongside the team.

    You will be working alongside our other 3 Liaison Nurses based in the team.

    This is a very rewarding role where you will bring your assessment, planning and problem solving skills together to work alongside people and their families to develop solutions around living well.

    You will need :

  • to be a New Zealand registered Nurse with a current practicing certificate
  • a driving license
  • a can-do attitude and a passion for working alongside people
