Our Early Supported Discharge team works to prevent avoidable admissions and to support timely transfer home for ongoing health needs assessment and recovery planning.
The team is expanding and we are looking for another Registered Nurse, keen to work with people in the community following presentations to our ED and Inpatient services.
Many of our patients are older adults so you will need a special interest in caring for this population group, completing comprehensive assessments and referring to colleagues / other services depending upon the needs of your patient.
We will soon be seeing people discharged home for rehabilitation following an injury, so you will also bring a restorative / rehabilitative approach to your management and rehab plans.
Our staff are our most valuable asset so we are committed to using their time well. Our future direction will see us delegating more work to highly trained Rehabilitation Assistants and, where appropriate, cross-skilling with other professions to reduce unnecessary referrals and duplication of work.
You will continue to find professional support from your nursing colleagues within the team and from the Associate Director of Nursing.

You’ll also bring sound clinical skills, excellent time and case load management, great communication, and inter personal skills.
The successful applicant will be joining a core team of two RNs and a Health Care Assistant. In the short-term the role will be 24 hours per fortnight, increasing to 40 hours as additional funding comes on line.
