Bring your exceptional clinical skills to the Nurse Case Manager role focusing on health management of our Pasifika patients and fanauwith high complex health needs!
Reporting to the Regional Pacific Senior Programme Manager you will be responsible for working closely with the Regional Pacific Team to provide clinical management of Pasifika patients who have been referred from overseas.
Assessment and support may be within the Hospital or Community.
You will work actively in partnership with the medical and nursing staff across the care continuum to ensure safe, patient and family centred care in line with the vision and values of Counties Manukau Health.
The Nurse Case Manager will be responsible to support individual patient pathways across the primary, secondary and tertiary care interface, establishing effective working relationships with members of the multidisciplinary team, patients, their families and care givers.
To be successful in this role you will be an experienced NZ Registered Nurse who is already functioning at a Competent or Proficient level on the PDRP with a passion for Pasifika Health.
This role will see you actively engaged to support better health outcomes for Pasifika patients referred from overseas. As such it is essential that you are fluent in a Pasifika language.
