Service : Early Supported Discharge (ESD) team

Hours : 32 to 40 per week
Our Early Supported Discharge (ESD) team is expanding and will now be supporting whānau by bringing people home earlier following a stroke.
This means we have positions available for a Physiotherapist who is keen to help us transform our health services in the community.
There will be full time role for Physiotherapy dedicated to ESD but we have some flexibility around hours and are interested in hearing from therapists who can work 32 to 40 hours.
You will work with our colleagues in the acute medical unit and the Older Persons and Rehabilitation wards to identify whānau who can beneficially progress their rehabilitation at home, initially with more intensive input from ESD, and then transferring to Community Rehabilitation as required.
Whilst this tranche of ESD development will have a particular focus on Stroke, the service to Older People and those discharging following an acute medical illness will continue to evolve.
We are also planning to incorporate post-injury rehabilitation as our third stream of intensive home-based rehabilitation.
The ESD service is embedded in Te Herenga Tangata, our Older Persons, Rehabilitation and Allied Health community team. This means you’ll have a number of Physiotherapist colleagues as well as the skills of an extensive team of Allied health professions, RNs and Doctors to draw upon.
We will be moving towards more inter-professional practice approaches and are looking for therapists keen to cross skill and make the best use of what will be a growing Assistant workforce.

You’ll also bring sound clinical skills, excellent time and case load management, great communication, and inter personal skills.

In order to offer a 7 day per week service, therapists in these roles will be rostered to work one weekend day per month with a rostered day off in the same fortnight and remuneration as per the MECA employment contract for Allied Health staff.
